PHOTOGRAPHY CONSENT AND RELEASE FORM FOR MINOR CHILDREN (Under 18)
I, the parent or official guardian of (child), hereby grant permission to Sunshine Bridge Education representatives, to take and use photographs and/or digital images of my child for use in:
1. Media releases, media articles – including newspapers, radio, television – printed publications and/or educational materials
2. Electronic publications and communications such as the club’s Facebook site and website
3. I agree that my child’s name (first name only) and identity may be revealed in descriptive text or commentary in connection with the image(s).
4. I authorise the use of these images without compensation to me. All negatives, prints, digital reproductions and shall be the property of Sunshine Bridge Education.

Sunshine Bridge Education’s General Terms and Conditions
I agree to abide by these terms and conditions and to ensure that all authorised persons who may attend the Centre on my behalf to drop‐off or pick‐up the Child will also abide with these terms and conditions.
1. Attendance
a. I delegate the custody and control of my Child to *SBE whilst SBE is caring for my Child and agree to accept the Employees and facilities that SBE provides to care for my Child.
B. I acknowledge that, although SBE warrants that it will at all times and to the best of its ability to use all reasonable care in respect of my Child, there are inherent and unforeseeable dangers and difficulties in providing the Services and SBE cannot guarantee the wellbeing of my Child at all times.
c. I authorise SBE to take any action it considers necessary, appropriate or in the best interests of my Child in the circumstances to protect my Child at the Centre or otherwise in its care or control
2. Fees and charges
a. I agree to pay all Fees payable for my Child during the Term.
b.I understand that SBE will email me a statement of the balance of my fees owing, unless I have signed up to Ezidebit.
c. I acknowledge that if I fail to pay the Fees on time SBE may cease to provide the Services to my Child.
d. Where my Child’s Enrolment has ceased, I understand that I can receive a refund of any credit balance on my account. Any refund will only be processed 3 weeks after any request in writing using the appropriate form provided by SBE to allow sufficient time for Third Party or other account adjustments to be completed. Refunds will be payable to a nominated account detailed on the refund form.
e. I acknowledge that I need to give SBE at least 4 weeks notice before exiting, and I will ensure fees are paid for these final 4 weeks, even if I do not intend to send my child. I acknowledge that I need to give SBE at least 2 weeks notice when dropping days, and I will ensure fees are paid for these days.
F. I acknowledge that there is a 10 percent charge for fees that are more than 2 weeks late, and that without a reconciliation after 2 months, my overdue fees will be sent to an appropriate agency outside of SBE.
5. Medical acknowledgements and consents
a. I understand that I am to provide current information (as required within enrolment form) in regards to any of my child’s medical conditions, including allergies and sensitivities, to the Centre on enrolment and continue to update this information with the Centre at least annually.
b. I consent to first aid being administered to my Child by any person who is the holder of a current first aid certificate.
d. If my Child’s temperature is at or above 38oC SBE is unable to contact me, the parents, guardians or emergency contacts listed on my Child’s Enrolment Form within a reasonable time.
e. I acknowledge SBE will inform me as soon as possible about the nature of the illness or injury to my Child.
6. Behaviour of Parents/Guardians
a. I agree that I have a responsibility to treat all SBE team members with respect and I agree to behave appropriately at all times when dealing with any Employee. Swearing, raised voices and rudeness directed at team members will not be tolerated and I understand my Child’s Enrolment may be terminated if I breach this provision.
b. I acknowledge the personal and sensitive nature and context of the Services and information provided in relation to the Services and agree to keep, and ensure that others keep, all correspondence, documents and/or other information provided to me by SBE or in relation to the Services confidential.
C. SBE reserves the right to refuse entry to any of its Services to anyone for any reason and at any time.
7. Behaviour of students
a. I am aware that if my student exhibits behavioural difficulties as mentioned in the 2019-2020 parent handbook, their enrolment at SBE may be discontinued.
8. Guarantees
a. I accept that Sunshine Bridge Education makes no guarantee of assessable progression. While SBE will do its best to provide quality education and advice to parents, in the forms of tuition and consultations, I agree that no guarantee has been made. If SBE finds itself unable to assist my Child, they will inform Parents and Guardians in due course.
9. Registration
a. I acknowledge that it is my responsibility to ensure that my child is either enrolled with Homeschool QLD, are in the process of enrolling, are enrolled in distance education, are enrolled in a State or Non-State school in QLD or NSW, or they are of preschool age.
10. While SBE can assist with planning, SBE is not responsible for parents and carers meeting this requirement.
10. Amendment of Terms and Conditions
a. I accept that SBE may change these Terms and Conditions without notice to me, provided that the updated Terms and Conditions are made available upon request. If changes to the Terms and Conditions would reasonably be expected to be material to me, I will be given no less than the Notice Period before such changes apply.
b. Parents and guardians who send their children to SBE more than 3 times without signing the above forms are still bound by these terms and conditions.

- I, the parent, give permission for my student/s to walk safely with their teacher/s to the park or Library at morning tea times for eating and play time/and or for social/holiday club.
- I will ensure my child is prepared for sun safety (sunscreen and hat at parent/guardian's disgression)
